Certified Midwife

certified midwife means an individual who holds a current and valid national certification as a certified midwife from the national midwifery certification board

Requirements

Complete and submit the Application for Licensure with the following information:

  • Appropriate fee made payable to the Maine State Treasurer or if paying by credit card complete the credit card authorization form.
  • Verification of Licensure (if needed)
  • Proof of successful completion of a graduate-level education program in midwifery that is accredited by the Accreditation Commission for Midwifery Education
  • Proof of a current and valid national certification as a certified midwife from the National Midwifery Certification Board

Annual renewal fee $275.00

Fill out the online renewal application and submit.

The Department must verify that you have met all the conditions for renewal before your license is renewed. Online submission of your renewal application should not be construed as automatic renewal of your license.

Requirements

  • A late fee of $50 is assessed for licenses renewed after the license expiration date.
  • A person who submits an application for renewal more than 90 days after license expiration date is subject to all requirements governing new applicants.
  • Renewal reminders are electronically mailed approximately 60 days prior to the license expiration date. It is the licensee's responsibility to keep the Board informed of the current e-mail address and/or to see that the license is renewed absent the renewal application.

Certified Professional Midwife

certified professional midwife means an individual who holds a current and valid national certification as a certified professional midwife from the national registry of midwives

Requirements

Complete and submit the Application for Licensure with the following information:

  • Appropriate fee made payable to the Maine State Treasurer or if paying by credit card complete the credit card authorization form.
  • Verification of Licensure (if needed)
  • Proof of successful completion of a formal midwifery education and training program as follows:
  •  An educational program or institution accredited by the midwifery education accreditation council;
  • For an applicant certified as a certified professional midwife who is certified before January 1, 2020 and who has completed a midwifery education and training program from an educational program or institution that is not accredited by the midwifery education accreditation council, a midwifery bridge certificate; or
  • For an applicant who has maintained an authorization to practice midwifery as a licensed certified professional midwife in a state that does not require completion of a midwifery education and training program from an educational program or institution that is accredited by the midwifery education accreditation council, regardless of the date of that authorization, a midwifery bridge certificate.
  • Proof of a current and valid national certification as a certified professional midwife from the National Registry of Midwives.
