Primary Care Plus (PCPlus) Application and Recertification Now Open: Action Required for All PCPlus Practices

The PCPlus application is now open and will remain open through Friday, May 1, 2026.

PCPlus is a MaineCare primary care model that supports Primary Care Providers (PCPs) by giving them greater flexibility and incentives to meet members’ needs. The model shifts from fee‑for‑service to population‑based payments tied to cost and quality outcomes. Participation is voluntary, and all PCPs must apply to enroll or remain enrolled.

One Application for New Enrollment or Recertification

This year we are using a single application. Through this application, practices indicate whether they are a new provider applying to PCPlus for the first time, or a current PCPlus provider completing annual recertification.

The following version of the application (in a “view only“ format) is available for:

  • New PCPlus Provider: Practices not currently enrolled in PCPlus use this option to join the program. The annual enrollment start date is July 1, 2026.
  • PCPlus Provider Recertification: Existing PCPlus practices complete this option to remain enrolled. All current PCPlus practices must complete recertification, even if they do not expect a tier change. Practices may request a tier adjustment based on updated practice characteristics.

Recertifying providers may also request a tier change if they now qualify for a different tier. Practices must submit a separate application for each service location.
