The Single Assessment

Medium to High Intensity Services

For families of children with behavioral health disabilities who are at risk of out-of-home treatment, accessing the right services can be confusing. The Single Assessment is a new process designed to transform how children and families access medium-to high-intensity behavioral health services. The Single Assessment establishes medical necessity, level of care, and service eligibility for each child. Using one of the three age-appropriate, standardized tools, Early Childhood Service Intensity Instrument (birth-5), Child and Adolescent Level of Care/Service Intensity Utilization System (6-17), Level of Care Utilization System (18-21), the assessment determines the level of service a child needs and identifies which services they are eligible to receive.

Beginning February 2026, the Single Assessment will function as the initial clinical authorization statewide for children seeking medium-to high-intensity services.
