2026 Creditable Coverage Notice

Effective January 2026: The State of Maine will no longer be mailing Form 1095-C to employees and instead will be digitally accessible within PRISM; however, employees and retirees may still request a copy if they do not have access to PRISM. To request a paper or digital copy of your 1095-C Form, please contact our office at info.benefits@maine.gov and utilize the subject line to indicate "1095-C Form Request", or call us at 207-624-7380, or mail us a request at Office of Employee Health, Wellness, & Workers' Compensation at 61 State House Station, Augusta, ME 04333-0061. Information on this form refers to tax statements reporting that individuals had health coverage.
