Title 5 Facility License Maintenance Case
Providers whose facility license is under a Title 5 review may use their Title 5 notice to extend their enrollment with MaineCare in MIHMS by six months, and for every six months thereafter, until the Title 5 review is resolved. Providers must complete this update by submitting a maintenance case via the Health PAS Online Portal. They can use the Provider Maintenance User Guide for step-by-step instructions.
Select the “Title 5 License Extension” from the licensing board drop-down menu and complete the maintenance case as usual. Providers can use the ALMS website to search for their license and must take a screenshot to upload with the maintenance case to show the Title 5 status.
Please ensure the "Term Date” is 6 months after the license expiration date. For example, if you submit your Title 5 review and your license ends on January 30, 2026, extend the “Term Date” out to July 30, 2026. As necessary, repeat this process every six months until the license is renewed. Once the license is renewed, submit another case with the new term date of the license.
Providers take note of your license expiration dates and plan to submit a maintenance case to update that license information a minimum of 30 days prior to the license expiration date. Be sure to account for this when planning and submitting other maintenance cases.
Note: MaineCare will not reimburse providers for services rendered when the they fail to renew and update their license within a maintenance case prior to its expiration.
