Single Assessment Timing Requirements Reminder

The summary below outlines the timing requirements for initial and repeat Single Assessments, including the limited exceptions to the standard annual assessment cycle for children receiving medium- and high-intensity behavioral health services.

 

Situation

When is a Single Assessment Required?

Timing

Child already receiving services (no prior Single Assessment)

If the child has never had a Single Assessment, complete one before the next authorization is issued. After that, follow the standard annual reassessment schedule.

Before the next authorization (typically at the next Continued Stay Review (CSR)), then every 12 months

New request for medium- or high-intensity services

Complete a Single Assessment before submitting the initial referral or authorization request.

Initial assessment

Child continues receiving medium- or high-intensity services

Repeat the Single Assessment to confirm ongoing eligibility. Continue submitting CSRs between annual assessments.

Every 12 months

Child residing in a Children’s Residential Care Facility (CRCF)

Complete a Single Assessment when discharge planning begins to determine eligibility for community-based services after discharge.

At discharge planning

On a CRCF wait list

Repeat the Single Assessment while the child remains on the wait list.

Every 6 months

Child receiving services that began before the Single Assessment process was implemented

If no Single Assessment has been completed and the current authorization is nearing expiration, complete an initial Single Assessment before the next authorization. After that, follow the annual reassessment schedule.

At the first CSR after Single Assessment implementation, then every 12 months thereafter.
