Reminder: Primary Care Plus (PCPlus) Application Closes Today

PCPlus is an adopted MaineCare policy that supports primary care practices in delivering high-quality primary care. We anticipate that this policy will have a future effective date of July 1, 2022.   

The PCPlus application is open for practices who have not yet applied. Practices that have already applied will not need to reapply. This PCPlus application period closes as of May 13, 2022. All documents required through the application, except for the Community Health Worker Environmental Scan and the Joint Population Health Strategy, must be submitted by May 13^th.
