Primary Care Plus (PCPlus) Model Advancement Meeting – Update

Please join us on Wednesday, March 25, 2026, for an update on service model planning work for PCPlus Phase II. This will be a follow-up to our August meeting where we provided an overview of the current PCPlus model, gathered initial feedback from participating practices, and discussed key priorities for the next phase of PCPlus. For your reference, you can access the Powerpoint presentation from the August meeting. 

During this upcoming session, in collaboration with our vendor, Manatt Health Strategies, we will share the developments since August 2025. 

This meeting will give us the opportunity to hear feedback from practices about the current design work as we strategize about the next phase of the PCPlus model. 

Discussion Topics: 

  • Review the guiding principles shaping the next phase of PCPlus to improve how primary care is paid for and delivered.
  • Provide updates about progress since the last meeting and outline potential approaches that continue to advance primary care toward an alternative payment model tied to quality and outcomes. 
  • Engage participants to provide feedback, offering time to share input on proposed strategies and identify areas of improvement.
