Service Notice: Temporary Pause on Personal Visit Scheduling
To accommodate a system upgrade, scheduling for personal visits will be temporarily unavailable.

Personal Visit Scheduling Pause: Scheduling will be unavailable from noon on Friday, September 25, 2026, until Tuesday, October 6, 2026.
Existing Personal Visits: All personal visits scheduled and confirmed before the September 25 cutoff will still take place as planned.
Recommendation: We highly encourage you to schedule any upcoming personal visits in advance to avoid disruptions.
Professional Visits: Professional visits will continue as normal. Professional visitors should contact the facility directly for assistance with scheduling.

This is a request for visit scheduling. You will receive a copy of the request after submitting the form. The visit is NOT scheduled until you receive confirmation from the Visit Office that the visit has been scheduled.

Note: Contact visits are limited to three (3) visitors for all resident housing areas. A resident may submit a Special Visit Request for additional seats. These requests must be submitted to the Unit Team at least two weeks in advance and are reviewed on a case-by-case basis.
