The Children and Youth with Specific Healthcare Needs Program's mission is to improve the health and well-being of children and youth with specific health needs by developing and sustaining community-based systems of care.

The CYSHN Care Coordinator is responsible for promoting the health of children and youth with specific healthcare needs through state-wide initiatives and collaborations. The Care Coordinator is a unique resource for families, providers, and communities. The goal is to assist families and providers in finding services that will help the child or youth acquire optimal health, development, well-being, and the best quality of life.

The care coordinator can assist with insurance appeals, application processes, advocacy, navigation of healthcare systems, referrals, finding resources, connecting families to other support systems and services, and training case managers/ care managers on best practices and guidelines.

This service is available to families of children and youth from birth to age 21 that have, or are at risk for, chronic medical conditions, behavioral and emotional conditions.
