Partners in Care Coordination
The Partners in Care Coordination Program (PCC) provides health, community information and referral information to families of children and youth from birth to 21 years of age.
The Partners in Care Coordination Program's goal is to help improve the health, development and well-being of Maine's children who have special health needs by offering solutions to what can sometimes seem insurmountable concerns.
PCC is also available to other agencies for assistance. Partners in Care Coordination assists families and providers in The State of Maine, to find supports, resources, and referrals to community services.
