The document request form is for requesting a printed copy of an electronic document produced by DHHS. For example, Primary Tax Credit Report for 2018 - Feb-2019 (PDF)

Personal documents like benefits, SNAP, birth, death or marriage certificates, tax forms, etc., cannot be requested through this form.

*Limit one (1) copy per document. Printed documents for Maine residents only.
