Go to https://www.maine.gov/boardofnursing - select I want to add or change a physician or NP supervisor and follow instructions. Supervisor MUST have ACTIVE MAINE license. If supervising provider is an APRN, he/she must have practiced as an APRN for a minimum of 5 years in the same specialty and worked in a clinical health care field for a minimum of 10 years.
