  • If yes, please provide the duration, the provider of the service and a brief description of the service: Kaizen, as YPD HCM, INC (“YPDâ€), became alerted to a data incident on or about January 23, 2025, when it learned certain information had been inadvertently printed on envelopes. Upon learning of the incident, YPD promptly moved to secure the incident and implemented additional security measures to help ensure the incident does not reoccur. Kaizen then undertook a comprehensive review of the data potentially impacted in this incident and determined certain personal information in the form of name and social security number had been inadvertently printed on the envelopes. Kaizen then took steps to notify potentially impacted individuals as soon as possible. Kaizen is providing credit monitoring services to the potentially impacted individuals for a duration of twelve (12) months through Cyberscout, a TransUnion company specializing in fraud assistance and remediation services.
