School Food Service Reviews and Review Information
There are several items needed to complete the review. Collecting the information ahead of time will cause less stress for you and a smother day of review.
Items that will be needed and asked for during the review are:
- Current year applications
- Direct Certification List (on web from DHHS)
- Current year master list of students indicating category
- Status change dates list or report
- List of employees by school
- Amount of scheduled employee hours for week or day
- Financial records for month of review
- unpaid bill listing
- disbursements
- income
- inventories
- a la carte income /expenses
- account balance
- Production records review period
- Menu analysis if available
- Recipes
- Daily record forms
for month of review
- lunch (generally will need a copy for us to take)
- breakfast
- snack
- milk
- Wellness policy / Guidelines
- HACCP plan
- School Year Calendar
- documentation of on site review to view
Please NOTE: Additional information ay be requested if problems or concerns are identified.
The review Process Generally is in this order but may vary depending on school and reviewer.
Reviewer will:
arrive at the school and review all applications on file for current year
compare applications to master list
observe kitchen sanitation
observe accountability system
review a la carte items for chapter 051 rule
review current production records
obtain meal counts by category for day of review
review financial records
exit conference with superintendent or delegate.
Other items may be requested by the reviewer as needed.
