Home > Data Center > Special Services Forms > EF-S-02 Excel Spreadsheet Instructions EF-S-02 Excel Spreadsheet Instructions: This version of the EF-S-02 Excel Spreadsheet is designed to allow the Office of Special Services to upload the Special Education program data for your school administrative unit. This spreadsheet is in MS Office 95, Excel version 5.0 and is available from the Department of Education homepage: http://www.maine.gov/education/forms.htm or you may contact the Tom Coulombe at the Office of Special Services at (207) 624-6650 for a diskette. Download the EF-S-02 Excel spreadsheet: a) Open your web browser (Microsoft Internet Explorer or Netscape Navigator). b) Go to http://www.maine.gov/education/forms.htm c) Scroll down until you see the form you need. d) Click on the Microsoft Excel icon next to the form name. This generally will download the Excel spreadsheet into your computer. You may see an option to Open or Save File. Choose Save File. · With some forms, clicking on the Excel icon takes you to another web page first. The form should be listed on this page. Please follow these instructions when submitting this form via e-mail or diskette: a) Open Microsoft Excel (or another spreadsheet program, such as Lotus 1-2-3). b) Open the spreadsheet form you have downloaded. c) Save the form with a different file name (Note: if using a spreadsheet program other than Microsoft Excel, make sure it has an option to save spreadsheets as Excel files with a .xls extension). d) Fill in the information on the form. Save often. · Do not use the Cut and Paste feature to move an amount from one cell to another - an amount should be deleted with the delete button and retyped into the correct cell. · Please do not delete the formulas within the cells; they are there for your convenience. Specific EF-S-02 instructions when submitting this form via e-mail or diskette: a) On sheet EF-S-02CVR in cell "H6" type in the name of your School Administrative Unit (municipality, S.A.D. or C.S.D.) This will place the name of your unit on each page. b) On sheet EF-S-02CVR in cell "F7" type in the person completing the form. c) On sheet EF-S-02CVR in cell "M7" type in the phone number of the person completing the form. Send the EF-S-02 spreadsheet to the Office of Special Services, Department of Education. a) Open your email program. b) Create a new mail to tom.coulombe@maine.gov. c) You will need to attach the spreadsheet to the email. In your email program, there should be an option such as Attach File. Select your completed form. d) Send the email! Print and send the signed EF-S-02 cover page to the Department of Education. Please send a signed, printed copy of the EF-S-02 cover page to the M.I.S. Team (at the address indicated on the EF-S-02 form). This copy should indicate whether this report is being submitted via diskette or e-mail. |
