TO: Superintendents, Special Education Directors, CDS Regional Site Directors FROM: Susan A. Gendron, Commissioner DATE: April 4, 2007 RE: Required State Forms For Special Education The Department has developed the following forms that will be required in all School Administrative Units (SAUs) and Intermediate Educational Units (IEU) [CDS Regional Sites] in the administration of their early intervention and special education programs beginning September 1, 2007:
The forms have been developed in keeping with the objective of paperwork reduction and in order to ensure that the State does not exceed the minimum federal requirements. Utilization of State forms will also ensure compliance with the federal regulatory requirements and consistent documentation throughout the State. These are standardized documents and must be used as they are; please do not modify them. Department staff will be training SAU staff on the utilization of these forms at the regional trainings to be held this spring.
