RE:       Survey – Public Health Work Group

The Maine Department of Education and David Noble Stockford, Team Leader and Policy Director for Special Services, serve on the Public Health Work Group (PHWG) and as an active member of this group, we are requesting your participation in an important survey about defining a Comprehensive Community Health Coalition as part of Maine’s future public health system.

The mission of the PHWG is to design the framework for Maine ’s comprehensive public health system.  The PHWG’s Coalition Core Competencies Subcommittee was charged by the Legislature – as reflected in the Maine State Health Plan – to make recommendations as to what functions, competencies, and performance standards should, in the future, be expected of Comprehensive Community Health Coalitions (CCHC) as part of Maine ’s developing public health system.

This survey is designed to give broad input on the future definition of a CCHC.  Please click on the following survey link to participate:

Responses will be accepted through July 14, 2006.  If you should have difficulty accessing or completing this survey, or have questions or comments, please contact Joanne Joy, Co-Chair of the PHWG Coalition Core Competencies Subcommittee at 207-582-8011.

Thank you, on behalf of the Public Health Work Group, for participating in this important survey.
