RE:                   2005 Summary Report on the Immunization Status of School Children – Kindergarten, Grade 1, Grade 7, and Grade 8

            The Maine Department of Education and the Maine Department of Health and Human Services are conducting the annual immunization assessment of all students enrolled in Kindergarten, Grade 1, Grade 7 and Grade 8.  This assessment is in accordance with the Maine School Immunization Law (Title 20-A MRSA Sections 6352-6359), which requires students enrolled in grades K-12 to be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, and polio.  Students in grades K-3 and grades 6, 9 and 10 are required to be immunized against varicella (chicken pox) as well.

            Enclosed is the “2005 Immunization Status Report” for your school.  The summary report should be completed for each school building that has pupils enrolled in Kindergarten, First, Seventh or Eighth Grade.  Please do not summarize an entire school unit (district) on one report form.

            Please return the completed Status Report by December 31, 2005, to the Department of Health and Human Services, Immunization Program, 11 State House Station, 286 Water Street, Key Plaza 9th Floor, Augusta, Maine 04333-0011.
