RE: 2004 Summary Report on the Immunization Status of School Children – Kindergarten, Grade 1, Grade 7, and Grade 8.
The Maine Department of Education and the Maine Department of Health and Human Services are conducting the annual immunization assessment of all students enrolled in Kindergarten, Grade 1, Grade 7, and Grade 8. This assessment is in accordance with the Maine School Immunization Law (Chapter 661, Public Laws of Maine, Title 20-A MRSA Sections 6352-6358), which requires students enrolled in grades K-12 to be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, and polio. This year, Kindergarten and Grade 1 students are required to be immunized against varicella (chicken pox) as well.
In the coming weeks, your school nurses will be receiving this letter along with the “2004 Immunization Status Report” for your school. The summary report should be completed for each school building which enrolls pupils in Kindergarten, Grade 1, Grade 7, and Grade 8. Please do not summarize an entire school administrative unit (district) on one report form.
Please return the completed Status Report by December 31, 2004, to the Department of Human Services, Immunization Program, #11 State House Station, 286 Water Street, Key Plaza 9 th Floor, Augusta, Maine 04333.
