SUBJECT:       Habitual Truant Report Form for the 2003-04 School Year

 

 

            Enclosed is a Habitual Truant Report Form (EF-I-25) for each school unit under your supervision.  If you have a school union and/or more than one school system under your jurisdiction, you must complete a form for each school unit.

 

            These forms are to be returned no later than October 1, 2004.
