With the exception of the EF-T-18, the forms listed below are available on the Department of Education’s website. These forms may be downloaded from the Forms Page at: http://www.maine.gov/education/forms/constrtransp.htm#trans All reports are due back to the Department no later than July 30, 2004.

The Public School Bus Inventory and Mileage Report is not available on line but was mailed to the Superintendent on June 18, 2004 as part of the full pupil transportation reporting package, which includes all instructions and forms listed above.

Please read the forms and instructions carefully. They have changed significantly from last year. The data provided on these reports will be used as baseline data to determine the Essential Programs and Services funding levels for 2005-06. All reports are due back to the Department no later than July 30, 2004. This is notification that subsidy checks will be withheld, beginning with the August check, if your reports have not been received.
