The enclosed Annual Financial Reports of Federal Program Expenditures - Local Entitlement Part B (5-21 year olds) EF-S-09E and Section 619 (3-5 year olds) EF-S-09E - are to be used to report final expenditures and program description (5-21 only) based on the local entitlement application (EF-S-08) submitted to, and approved last fall by, the Office of Special Services.

The two enclosed EF-S-09Es for Part B and Section 619 consist of:

Part I Annual Financial Report of Federal Program Expenditures

Part B and Section 619

Part II Affidavit of Superintendent (Fiscal Agent)

Part B and Section 619

Part III Narrative Report

Part B ONLY

The Annual Financial Report of Federal Program Expenditures serves as an audit report of P.L. 105-17, Part B and Section 619 funds actually expended by June 30, 2003. Failure to submit this report by July 31, 2003 could delay future local entitlement payments.

If you have any questions relating to the EF-S-09E, please contact John Kierstead or Tom Coulombe at 624-6650 for Part B, and Brenda Wilkins at 624-6660 for Section 619 Pre-School.
