The attached page(s) represent school system data on file
at the Department of Education. Please
verify the data, sign and return the original to the Management Information
Systems Team before August 15, 2002.
You may then notify us of any additional changes anytime during the year
as they occur by contacting Shirley McQuarrie shirley.mcquarrie@state.me.us

out incorrect data and write in the corrected data above.

If
a school is not listed, add it to the end of the listing.
3.
If
a school has closed, cross out the entire line of data.

closed but moved to a different building (new or otherwise), please
indicate that. In many cases, it will
be necessary for us to adjust the coding.]
DEFINITIONS:
EMAIL ADDRESS -- We encourage you to provide a generic email address that does not
include an individual person's name.
This offers a more stable address, especially with the turnover of
personnel. Also it provides one address
that could be used for the entire central office.
GRADE SPAN - The first two digits
indicate the lowest regular grade taught and the second two digits indicate the
highest regular grade taught. If the
school has a self- contained special education class, the highest grade taught
is followed by an 'S'. If the school
has a 4-year-olds' program and/or early kindergarten, the highest grade taught
is followed by a 'P'.
CODING - State Department Use Only.
NAME OF PRINCIPAL - School principal's full name including middle initial, as it will be
reported on the Staff Information System (EF-M-15) this Fall for the 2002-2003
school year.
SCHOOL TEL. NO. - Use the primary contact
number.
FAX NO.
- Use the primary contact number.
