RE:                   Application for Approval of Substitute Teachers

 

 

        Enclosed is a supply of Applications for Approval of Substitute Teachers (EF-C-306), to be used during the 2001-2002 school year.  Please submit an original and one copy of the completed form.  Additional forms may be obtained through this office or you may make photocopies, if you desire.

 

        Please make sure the form includes correct names, addresses, and social security numbers.  Substitutes holding a valid Maine certificate, authorization, or approval do not need to be listed on the enclosed form.
