            The enrollment data collection forms listed below are now available on the Department of Education’s Web page. These forms may be downloaded from the Forms Page at:

1.      Report of Adult Education for Subsidy Purposes January 1, 2001 - June 30, 2001 (EF-M-39A).  Due Date:  July 15, 2001

 

2.      Report of Public/Selected Private School System for Year Ending June 30, 2001 (EF-M-35).  Due Date:  July 15, 2001

 

3.      Post Secondary Enrollment Options Act Annual Report Fiscal Year July 1, 2000 - June 30, 2001 (EF-M-44).  Due Date:  July 15, 2001
