CLE Application: NHTSA’s “DWI Detection and Standardized Field Sobriety Testing” Student Course

PDS will provide lunch at this training. We will make every effort to accommodate common dietary restrictions. Do you have any dietary restrictions?

Food Selection

I certify that I have reviewed the agenda for this training and, if selected, I will attend all instructional time. I understand that space is very limited and that PDS incurs food costs for each person who registers to attend. If I am unable to attend, I will immediately notify PDS via email to darcy.fisher@maine.gov so that someone from the waitlist may be invited to attend.

I certify that if I am selected to attend the training but do not attend it in its entirety, this may affect my eligibility to attend future PDS CLEs.
