- Late renewal fee is $300.00
- Initial application, examination fee is $300.00
- A license may be renewed up to 90 days after the date of expiration upon payment of a late fee of $300.00 in addition to the $600.00 annual license fee.
Late renewal fee
A license may be renewed up to ninety (90) days after the date of expiration upon payment of a late fee of $300.00 in addition to the $600.00 annual license fee.
PLEASE NOTE: Licenses expire upon the expiration date of the license. Practicing with an expired license may result in disciplinary actions. The sole purpose of the 90-day period is to permit expedited processing of renewals. It does not in any way operate to extend the expiration date of your license to practice.
