How to Apply for Services

Before completing an application, please review the Vocational Rehabilitation Orientation Video and the Client’s Rights and Responsibilities document on this web page. Then download the fully accessible and fillable application to your computer and complete the application. Once you have completed the application, print it out and send it through the mail to a Bureau of Rehabilitation Services office near you. DVR is within the Bureau of Rehabilitation Services (BRS). If you do not have access to a printer, contact your regional BRS office and ask them to mail you an application.
