2.  Find out what times have not yet been booked.  Also note that DOE reserved time runs from approximately the beginning of September to Mid May.  Please check the table below for the weekly ITV times.

3.  You will be FAXED an informational package containing nine important documents.
A.  You will be FAXED or E-mailed, a "Confirmation Letter (Sample)" with the requested dates and times that must be checked for accuracy and faxed back to 624-6601.  (Note that there is a $25.00 fee if you cancel you programming less than ONE month from the broadcast date.)
B.  You will be FAXED, E-mailed, or may download the "UNET/ITV Basic Information Form" in WORD 2000 format, which must be received by Bob McIntire two weeks before the scheduled event.
C.  You will be FAXED, E-mailed, or may download the "D.O.E. Presenters Guide" in WORD 2000 format, which contains information necessary to successfully present your program.
D.  You will be FAXED, E-mailed, or may download the "DOE/UNET Presentation Agenda Form" in WORD 2000 format, which must be received by Bob McIntire two weeks before the scheduled event if your scheduled presentation contains multiple presenters, activities, or support materials.  IF you do not bring a copy of the agenda form to your scheduled ITV presentation the room will be set-up in the standard fashion.
E.  You will be FAXED, E-mailed, or may download the "Request for Authorization to use Audiovisual Material Form" in WORD 2000 format, which must be received by Bob McIntire two weeks before the scheduled event if you will be using video cassettes.
F.  You will be FAXED, E-mailed, or may download the "DOE ITV Video Playback Form" in WORD 2000 format, which must be received by Bob McIntire two weeks before the scheduled event if you will be using video cassettes.
G.  You will be FAXED, E-mailed, or may download the "DOE/ITV Program Duplication Request Form" in Adobe Acrobat format, which can be used to order copies of the desired programming.
H.  You will be FAXED, E-mailed, or may download the "Local Videotape Productions Authorization and Release Form" in WORD 2000 format, which must be received by Bob McIntire two weeks before the scheduled event.
I.  You will be FAXED, E-mailed, or may download a listing of the "High School ITV sites and /or Sites Potentially Available to DOE Program Viewers" in WORD 2000 format, which can be used to contact sites to determine if resources are available on the requested dates.  IMPORTANT:  YOU must decide which sites you wish to broadcast to and contact these sites directly to determine if resources are available on the requested dates.
