The forms listed below are available on the Department of Education’s web site. These forms have been converted from paper/Excel format to web-based reports.
EF-M-40 Fall School Statistical Report
EF-M-35a Report of Public/Selected Private School System Part II -
Dropouts
These reports must be completed as of October 1 and returned no later than October 15, 2004. Please go to the following internet link and click on the appropriate form to complete the information for your schools/school district(s):

NOTE: To access the web forms, you will need to use the same superintendent login ID used for the School Lunch Reimbursement System.
Revisions
to the above reports are due to the No Child Left Behind
Act (NCLB) that requires each state to identify (1) graduation data, (2) dropout
data, and (3) aggregate daily attendance and membership by ethnicity and gender
as well as in the following four sub-groups:
o Migrant Education students
o Limited English Proficient students
o Special Education students
o Economically Disadvantaged students (Free & Reduced Lunch)
In order
to meet these federal requirements, the Department of Education has modified
the 2004-05 Fall School Statistic Report (EF-M-40) and the 2003-04 Report of
Public/Selected Private School System – Part II Dropouts (EF-M-35a) to include
the four sub-groups and they have become web-based reports
