Education Disruption > Forms FormsThe 2008/09 Model Form Maine Department of Education Diploma for Students Experiencing Education Disruption: Is used for those students grades 10-12 who experience education disruption for 10 or more consecutive days as defined in Public Law 451 and have been either identified by their local school unit or notified that they will not be earning a local high school diploma, and are within a few credits of meeting their local high school graduation requirements. This form contains model School Work Recognition Plan and Academic Waiver Forms as required by Chapter 451, and the Recommendation & Checklist for the Maine Department of Education Diploma. Model Form Students Experiencing Education Disruption: Is used for those students grades K-12 who experience education disruption for 10 or more consecutive days as defined in Public Law 451. This form can be used for those secondary students (grades 9-12) who are working toward a local high school diploma and are on track to graduate with their class or on schedule. This form contains model School Work Recognition Plan and Academic Waiver Forms as required by Chapter 451. |
