INSTRUCTIONS FOR COMPLETING FORM EF-T-23 

Purpose of Form EF-T-23: This form provides a template for a “Letter of Intent” to be used by any SAU to confirm their commitment to purchase one or more school buses through the State of Maine School Bus Purchasing Program. Once submitted by the SAU, the information will be used by the Department of Education and the State Division of Purchases to confirm the number of buses to be purchased through each participating vendor. The information will also be uses as one factor for selecting purchases to audit for vendor pricing compliance and district satisfaction.

Due Date: The District must submit a completed EF-T-23 to the Department prior to issuing a purchase order to a vendor. Any exception to this due date must be approved by the Department prior to the date of a late submission.

Instructions:

  1. Completed forms must be received by the Department of Education prior to the district issuing a purchase order to the vendor. Faxes are accepted – fax number – 207-624-6841.
  2. SAU: enter the name of the school administrative district placing the order (e.g. SAD 3; SU 52, Winslow; Augusta Public School; 5 Town CSD, Rockport)
  3. Contact Person: enter the name of the person within the SAU who can respond to any questions about the intended school bus purchase.
  4. Contact Phone, Email, and Fax: Enter the phone number, email address and fax number for the Contact Person.
  5. In the first paragraph of the letter enter the name of the SAU, the number of school buses to be ordered and the school year in which the order will be placed and delivery made. For example: This letter is an official commitment by Waterville Public Schools to purchase 3 school buses through the State of Maine School bus Purchasing Program for School Year 2002-2003 as follows:……
  6. Vendor: Enter the name of the approved vendor that you have chosen to buy the bus(es) from. If you intend to buy from more than one Vendor, you must complete a separate EF-T-23 for each Vendor.
  7. Number of Buses Committed to Order: enter the number of buses in each category listed that you intend to order. Do not duplicate between categories. The superintendent or designee must sign and date the form.
