The Data, Research, and Vital Statistics office (Vital Records) at 220 Capitol Street in Augusta, Maine will be closed to walk in customers on October 16 & 17, 2014 due to the completion of necessary repairs to the sidewalk and bridge leading into the office building. During this closure, vital records can still be ordered through the mail or by contacting VitalChek online at www.vitalchek.com or by phone at 1-877-523-2659. Staff will also be accepting applications from the general public for copies of vital records in the parking lot at 220 Capitol Street during the closure. However, please note that these records will not be immediately available, rather requests will be filled within 48 hours and mailed to the requestor. We apologize for any inconvenience this may cause. Note that the office will also be closed on Monday, October 13 for the Columbus Day holiday.

Meaningful Use Stage 2 - Registration for Ongoing Submission

The Stage 2 measures for public health reporting state that an Eligible Hospital (EH) or Eligible Provider (EP) must register intent to submit ongoing submission of electronic data to the Public Health Authority (PHA) for Immunization Registries, Syndromic Surveillance, Reportable Lab Results, Cancer Registries, and Specialized Registries.
