The Maine version of the IACA approved UCC Information Request form is in a "fillable" format
in Adobe Acrobat (.PDF). You need to download a version of the Adobe Reader by visiting the Adobe website to use this form.
To use this "fillable" form, simply complete onscreen, print
and mail or fax to our office.
