National UCC Forms

 

The Maine Secretary of State will only accept for filing the UCC forms that were most recently adopted by the International Association of Commercial Administrators. These forms are provided below in Acrobat (.PDF) format. If you wish to complete the forms on-screen, you need to download a version of the Adobe Reader by visiting the Adobe Website.  To use one of these fillable forms, simply complete on-screen, print and mail to our office together with the proper filing fee.

Please note: The UCC-1 and UCC-3 Additional Party Form may only be used in conjunction with either the UCC-1 or UCC-3 form to add multiple debtors or secured parties.

In order to receive an acknowledgment of your filing, you must complete items A & B on the National Form.

If you have provided our office with an additional copy or a postage paid envelope in the past, please do not submit them with your filings. Thank you.
