"Highly Qualified" Teachers and Paraprofessionals Data Survey Instructions, 2012-13

Data for the 2012-13 “Highly Qualified Teacher” (“HQT”) Data Survey will again be web-enabled through MEDMS. In order to facilitate the Department’s reporting to the U.S. Department of Education, data from all school units should be submitted via MEDMS no later than March 30, 2013. The “HQT form” in MEDMS will be available on February 28, 2013.

This data survey is for 2012-13 school year data only.

Once data are collected for each school, an authorized central office designee will need to enter the data. In order to access the "HQT form" in MEDMS, the user will need to have the "HQT" role assigned to him or her.

The local MEDMS administrator with the “SAU Admin” role already has this permission and can assign it to other users. The survey is accessed in MEDMS from the Application menu option, “Forms.” From the Action menu, select: “HQT form”.
