RE:                  Parent Contact Information for Parent Survey

** Time Sensitive ** Please read immediately. A response is required on or before April 28, 2006.

The Department will be sending surveys to ALL parents of students with IEPs prior to the end of the current school year. In order to accomplish this in the allotted time, it is critical that we have contact information for all special education students.

Please direct the person responsible for maintaining your student contact information database to provide the following information electronically to George S. Smith, Ph.D. at smithphd@pivot.net.  George is assisting the Department with the conducting of this survey.

1.      Student’s Name
2.      Student’s Grade
3.      Name of the parent with whom the student lives
4.      Parent’s Street Address
5.      Parent’s Town
6.      Parent’s State
7.      Parent’s ZIP Code

All contact information should be forwarded to the above no later than Friday, April 28, 2006.
