Permanent Application

Dear Applicant:

Thank you for your interest in permanent Maine licensure. The Board participates in the Federation of State Medical Boards Uniform Application (UA) process. The UA benefits physicians by reducing redundancy in filling out multiple applications when applying for licensure in multiple states, thus increasing portability.

Click on the "Start Here" link below to start your Uniform Application. Please follow the instructions included with the UA, also noting the Board's policies regarding applications Policies re Applications and references Permanent Application Reference Requirements.

As noted in the UA instructions, you are not required by the Maine Board of Osteopathic Licensure to utilize the Federation of State Medical Boards FCVS (Federation Credentials Verification Service) service although the Board will accept the packet should you choose to do so. If you do opt to use it, please be sure to indicate that the packet needs to be transmitted to the Maine Board of Osteopathic Licensure.

Once you have submitted your Uniform Application (which is transmitted online to this office) and mailed your completed Affidavit, Addendum and the application fee of $350 (Note: Application fees are non-refundable). Pursuant to Board policy, please wait 30 (thirty) days before requesting a status update, or for an account of documents missing from your file. Inquiries are to be made via e-mail to osteo.pfr@maine.gov. Please allow between 60-90 days total for the processing of your application.
