File a Complaint

Once licensed, each physician is subject to review by the Board. Anyone may report to the Board his/her belief that a licensed physician has violated the statute or a standard of professional conduct by writing a letter or submitting a form supplied by the Board.

You mayfile a complaint by:

1. submiting the complaint form (PDF Document) electonically;

2. printing a complaint form (PDF Document) and/or an authorization form (PDF Document) here using Acrobat Reader and sending it through the mail;

3. requesting a form be sent to you by calling the Board's Consumer Assistant at 207/287-3608; or

4. by writing a letter of complaint to the Board at the address shown at the top of this site.

When information is received that a licensee may have engaged in conduct which is a ground for disciplinary action, the Board has the power to investigate. Depending on the results of the investigation, it may: find that there is no merit to the complaint or report and dismiss the matter; invite the licensee and complainant to attend an informal conference; enter into a consent agreement with the licensee; hold a hearing; or impose discipline allowed by 10 M.R.S.A. sec. 8003 and 32 M.R.S.A. sec. 2591-A, which may include suspension or revocation. The Board may also issue non-disciplinary letters of guidance.
