Case Coordination Unit
The Case Coordination Unit (CCU) started in 2016 as a specialized team-based case management resource. The CCU provides Maine residents simple and timely solutions to complex healthcare, financial, and placement issues through integrated provider and community involvement.
The CCU accepts cases based on referrals and prioritizes:
- Individuals at risk to themselves or others
- Adults who have been in the Emergency Department more than five days
- Other complex cases sent by the Governor’s and/or DHHS Commissioner’s Office

Emergency Department Collaborative
In 2010, the Department of Health and Human Services implemented the Emergency Department (ED) Collaborative to reduce avoidable and costly trips to the ED by connecting members to more appropriate care. The ED Collaborative connects to targeted care management services, provides education to providers on implementing program processes, and provides support to members in becoming more engaged in their own wellness. The ED Collaborative currently works with all hospital EDs in the state.
