IV. NARRATIVE
The chain guard was replaced on the #1 finish machine and was not aligned properly causing the chain to wear through the new guard. The resulting hole spilled approximately one gallon of lube oil to the ground. The spill was cleaned up using sorbent material and the guard fixed. No site visit was made.
