Compensation Rate
Enter the employee's TTD rate (based on his/her pre-injury AWW)

Enter the employee's current hourly rate of pay (hourly wage for hours worked while partially incapacitated)

Dependents
Determine employee's number of dependents. The number of dependents does not include the employee. "Dependents" equal exemptions, as reported on federal Tax Form 1040 minus one. For example a single taxpayer with no spouse or children has "0" dependents in most instances. The table entry for "0" dependents includes one exemption for the employee.
Tax Status
Determine the employee's tax filing status.
Determination Date (mm/dd/yyyy)
Enter the date the determination was made. This will effect the maximum weekly compensation rate used for this determination.
