If you are legally authorized to claim property belonging to a deceased person, such as the Personal Representative of their estate.

If you are legally authorized to claim property belonging to a living person, such as a Power of attorney.

If you are the listed joint owner of a property and the other owner is deceased.

If you are a holder and need more time to file your annual report, or would like an exemption from our electronic reporting requirement, please complete the Extension and Exemption Request.

Unclaimed Property Report of Owners
If you have received an exemption for reporting electronically you may use this form for your owner information.

Holder Reimbursement Request
If you are a holder claiming previously reported property, please complete the Holder Reimbursement Request.

A holder must attempt to find the rightful owner of property before the property is remitted to the state.
