Anthem Vision Open Enrollment

The State Employee Health Commission announces the Open Enrollment for the Anthem Vision Plan is going on from now through November 13th. (Please note: Anthem Vision is an entirely separate plan from your health insurance plan).

Open enrollment is the period of time in which you are allowed to:

Enroll in the vision plan if you do not already have our vision insurance
Cancel your existing vision coverage
Change your existing vision coverage (e.g. add or delete dependents)

If you do not have any changes to make and want to keep the coverage the same as it is now, you do not need to complete an application. Applications received by Employee Health & Benefits after November 13th will not be accepted. All changes made during open enrollment will take effect on January 1, 2010.

The biweekly pretax premiums effective January 1, 2010 are:

Employee only = $2.73
Employee + 1 family member = $4.38
Family = $7.11
