Minor Change Request

This form is used to request a minor change related to a previously issued LURC permit. Minor changes include transferring the ownership of a parcel or changing the permit holder, changing the phrasing of the Commission's written decision, making corrections to dimensions or setbacks, or making minor changes or expansions affecting less than 10% of an approved project. Download the application, print and fill out the form, and mail to the Commission with the appropriate fees and exhibits.
