You will find the Special Purpose Private School Annual School Approval Report form for the 2010-2011 school year located at http://www.maine.gov/education/forms/specialpurposeprivreport.doc

Please complete and return the original report form no later than July 1, 2010. Schools with outstanding certification violations, lacking the required documentation or failing to meet the deadline for submission will receive a change in the school’s approval status from Full Approval to Provisional Approval in accordance with Maine Department of Education Regulation 101 Section XII.2.B(2).
