Home > Rules & Legislation > Letters from the Commissioner > Informational Letter 074 2009 Summary Report on the Immunization Status of School ChildrenINFORMATIONAL LETTER: 74 TO: Superintendents of Schools and School Nurses The Maine Department of Education and the Maine Department of Health and Human Services are conducting the annual immunization assessment of all students enrolled in schools. This assessment is in accordance with the Maine School Immunization Law (Title 20-A MRSA Sections 6352-6359), which requires students enrolled in grades K-12 to be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, polio and varicella (chicken pox). This year the “2009 Immunization Status Report” is available electronically at http://www.maine.gov/education/sh/immun2009schoolreport.doc . Please note that the summary report should be completed for each school building. Please do not summarize an entire school unit (district) on one report form. Status Reports should be completed and returned by March 15, 2010, by e-mail attachment to Sherri Hallett at sherri.hallett@maine.gov or by mail to the Department of Health and Human Services, Immunization Program, 11 State House Station, 286 Water Street, Key Plaza 9th Floor, Augusta, Maine 04333-0011.
