SUBJECT: Private School Annual School Approval Report

You will find the Private School Annual School Approval Report form for the 2009-2010 school year located at www.maine.gov/education/forms/learningopportun.htm#approv . The school approval process includes a review of the MEDMS certification violation reports (for non-NEASC accredited schools) and a review of the required documentation. This year I am requesting that all of the required documentation be submitted. Schools that have not corrected certification violations, do not submit the required documentation or do not meet the submission deadline will have their school approval status changed to Provisional Approval in accordance with Title 20-A M.R.S.A. Section 2904. Please complete and return the original school approval report form no later than July 1, 2009 to:
