Update on Required State Forms

SUBJECT: Update on Required State Forms

The Department has developed two additional special education forms that are required for determining a child’s eligibility for Speech or Language Impairment services and one that is optional for recording the classroom observation. These new forms are:

  • Severity Rating Scales/Guidelines for Speech/Language Communications Services
  • Speech/Language Eligibility Criteria
  • Teacher Input/Articulation, Language and Voice (optional, helpful for the classroom observation)

The new forms, along with those listed below, can be accessed electronically at: http://www.maine.gov/education/forms/specservices.htm . Please reference the date at the bottom of each form for the most current revision. Additions and corrections to the sample forms are also now available at the web address listed above.

  • Individualized Education Program (IEP) (revised, based on Legislative Action, June 2007)
  • Individualized Family Service Plan (IFSP)
  • Written Agreement for IEP Amendment
  • Documentation for Excusal of IEP Team Member Whose Curriculum Area Is Being Discussed
  • Documentation of Agreement for Non-Attendance of IEP Team Member Whose Curriculum Area Is Not Being Discussed
  • Written Notice (revised, based on legislative action, June 2007 and to reflect additions for CDS meetings)
  • Summary of Performance
  • Advance Written Notice of I.E.P./I.F.S.P. Team Meeting
  • Parental Consent for Evaluation
  • Learning Disability Evaluation Report

The forms have been developed in keeping with the objective of paperwork reduction and in order to ensure that the State does not exceed the minimum federal requirements. Utilization of State forms will also ensure compliance with the federal regulatory requirements and consistent documentation throughout the State. These are standardized documents, must be used as they are and cannot be modified per individual choice.
