RE: 2007 Summary Report on the Immunization Status of School Children – Kindergarten, Grade 1, Grade 7, and Grade 8

The Maine Department of Education and the Maine Department of Health and Human Services are conducting the annual immunization assessment of all students enrolled in Kindergarten, Grade 1, Grade 7, and Grade 8. This assessment is in accordance with the Maine School Immunization Law (Title 20-A MRSA Sections 6352-6359), which requires students enrolled in grades K-12 to be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, polio, and varicella (chicken pox).

A hard copy of the “2007 Immunization Status Report” for each of your schools will be sent directly to school nurses via U.S. Mail, along with this Informational Letter. The summary report should be completed for each school building that has pupils enrolled in Kindergarten, Grade 1, Grade 7 or Grade 8. Please do not summarize an entire school unit (district) on one report form.

Please return the completed Status Report by December 31, 2007, to the Department of Health and Human Services, Immunization Program, 11 State House Station, 286 Water Street, Key Plaza 9th Floor, Augusta, Maine 04333-0011.

For additional information or assistance, contact the Immunization Program in Augusta at 1-800-867-4775.

Thank you for your assistance in this matter.
